Upon setting up the appointment, you will need to download, print and complete the documents listed in the intake packet that associates with your needs. Please bring the completed documents with you to the first appointment. In order to review these documents, you will need to download a version of Adobe Reader.

Fee schedules will be agreed upon prior to the first appointment. Please review the cancellation policy below for associated fees for late cancellations and/or no shows.

You will be responsible to pay directly to me the co-pay and deductible amounts associated with your benefit program. All health insurances will be billed on the day of service. If your insurance plan is not listed above or you do not know what your benefit plan states, please call the customer service phone number on the back of your insurance card to obtain information about your “in-network mental health benefits” or the cost of your “out-of-network mental health benefits” and reimbursement schedule. If your insurance is out-of-network, you will be responsible for the associated fees. Clients are responsible for payments at the time of service or as agreed upon between us.
Because I am not able to be paid when an appointment time cannot be filled, a twenty-four hour cancellation notice is required to alleviate charging you directly for a missed appointment. This policy and the charge fee will be discussed at the first session. Extenuating circumstances and undue hardship will be taken into consideration. Payment for service missed will be due upon the next visit.
